INVITATION FOR PRE-QUALIFICATION

1. INTRODUCTION
The Nnamdi Azikiwe University Teaching Hospital, Nnewi, was established to deliver specialized health services in a timely, effective, efficient, and economic manner as well as provide for training and research. The FGN has allocated public funds for the provision of radio diagnostics centre under its 2024 capital projects. Experienced and reputable contractors are hereby invited to submit bids for the following procurements:

2. SCOPE OF WORKS

RDC-01 Construction of MR1 building
RDC-02 Construction of digital X-ray building
RDC-03 Construction of access road for the radiology buildings
RDC-04 Construction of access road for the utility areas for the radio-diagnostics center
RDC-05 Construction of security infrastructure (security building) for the radio-diagnostics center to include surveillance system   (CCTV)
RDC-06 Construction of offices and other utility buildings for the radio-diagnostics center
RDC-07 Construction of ICT center for the radio-diagnostics center to include PACS system

3. ELIGIBILITY REQUIREMENTS
Suitable qualified companies are invited to submit three (3) bound copies of pre-qualification documents duly signed, paginated, separated by dividers and arranged in the order outlined hereunder:
a. Evidence of certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including form CAC1.1 or CAC2 and CAC7
b. Evidence of Company’s Income Tax Clearance Certificate for the Last three (3) years valid till 31st December, 2024; with a minimum average annual turnover of N100m (as stated in the TCC & concurrent with the audited accounts)
c. Evidence of pension Clearance Certificate valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 15 and above);
d. Evidence of Industrial Training Fund (ITF) Compliance Certificate Valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is ₦50Million and above);
e. Evidence of Nigerian social Insurance Trust Fund (NSITF) Clearance Certificate Valid till 31st December 2024;
f. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2024 or valid Certificate issued by BPP;
g. Sworn Affidavit:
 Disclosing whether or not any officer of the relevant committees of the Nnamdi Azikiwe University Teaching Hospital, Nnewi or Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;

h. Company’s Audited Accounts for the last three (3) years -2021,2022,2023;
i. Reference letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
j. Company’s profile with the Curriculum Vitae of key Staff to be deployed for the project, including copies of their Academic/ Professional certificates such as COREN, QSRBN, ARCON, CORBON, SURCON, NITDA & CPN, etc.;
k. Verifiable documentary evidence of at least three (3) similar jobs (in size, nature and complexity), executed in the last five (5) years including Letters of awards, Valuation Certificates, Job Completion Certificates and photographs of the project;
l. For supply of equipment: letter of authorization as representative of the original equipment manufacturers (OEMs);
m. RDC-07: evidence of registration/accreditation with the Computer Professional Registration Council of Nigeria and National Information Technology Development Agency;
n. Works: List of Plants/Equipment with proof of Ownership/Lease Agreement;
o. For joint Venture/ Partnership, Memorandum of Understanding (MoU) indicating lead Partner should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner)
Note: All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Headed Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM NO.) and e-mail address. The Letter head Paper must bear the names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.

4. SUBMISSION OF PRE-QUALIFICATION DOCUMENTS
Prospective bidders are to submit pre-qualification documents for each of the lot desired; two (2) hard copies each of the technical bids packaged in sealed envelopes and clearly marked as “technical bid”; and addressed to chief medical director nnamdi aizkiwe university teaching hospital, nnewi and clearly marked with (the name of the project and the lot number0. Furthermore, the reverse of each sealed envelope should have the name and address of the bidder and drop in the designated tender box located at the hospital’s registry unit not later than 10:00am Monday, 23rd December, 2024.

5. OPENING OF PRE-QUALIFICATION DOCUMENTS
The pre-qualification documents will be opened immediately after the deadline for submission 10:00am Monday, 23rd December, 2024, at nnamdi azikiwe university teaching hospital, nnewi, onitsha old road, Anambra state in the presence of bidders or their representatives. Please, ensure that you sign the bid submission register, as the nnamdi azikiwe university teaching hospital, nnewi, will not be held liable for misplaced or wrongly submitted bids. 6. GENERAL INFORMATION
a. Bids must be in English language and signed by an official authorized by the bidder
b. Bids submitted after the deadline for submission would be returned un-opened;
c. All costs will be borne by the bidders;
d. Only pre-qualified bidders will be invited at a later date for collection of tender documents;
e. The prequalification documents will be opened in the presence of bidders or their representatives, CSOs in the areas of anti-corruption and transparency and private professional bodies;
f. The nnamdi azikiwe university teaching hospital, nnewi is not bound to pre-qualify any bidder and reserves the right to annual the procurement process at any time without incurring any liabilities in accordance with section 28 of the public procurement act, 2007.
