INVITATION FOR TENDER/PRE-QUALIFICATION/EXPRESSION OF INTEREST (EOI) FOR THE NATIONAL EYE CENTRE KADUNA 2025 CAPITAL PROJECTS

NATIONAL EYE CENTRE, KADUNA

A. INTRODUCTION
The National Eye Centre Kaduna, a tertiary health institution under the Federal Ministry of Health and Social Welfare, in implementation of her 2025 Appropriation Act for Capital Projects and in compliance with the Public Procurement Act, 2007 hereby invites all interested, qualified and reputable contractors with relevant experience and track record of excellent previous performance to Bid for the following projects:

B. PROFILE OF PROJECTS – WORKS, GOODS & SERVICES
LOT NO / DESCRIPTION
LOT 1: INSTALLATION OF ALTERNATIVE POWER SUPPLY AND END-USE DEVICES: PROCUREMENT OF CNG 350KVA, INVERTER ACS, SOLAR STREETLIGHTS WITHIN THE HOSPITAL COMPLEX TO ENHANCE SECURITY, ENSURE REDUCTION IN POWER CONSUMPTION AND PROVIDE CLEAN ENERGY.
LOT 2: PROCUREMENT AND INSTALLATION OF HT ELECTRICAL CABLES FOR EXISTING SUB-STATIONS FOR ADEQUATE DISTRIBUTION OF POWER FROM INJECTION SUBSTATIONS TO POINT OF USE TO REPLACE OLD AND WORN-OUT UNDERGROUND CABLES.
LOT 3: REHABILITATION OF WATER SUPPLY AND RETICULATION SYSTEM OF 30-YEAR-OLD GROUND AND OVERHEAD STORAGE TANKS, SUPPLY AND INSTALLATION OF WATER TREATMENT PLANT, SUBMERSIBLE PUMPS, PIPE REPLACEMENT
LOT 4: CONSTRUCTION OF MAIN ENTRANCE GATE, OBSERVATION TOWERS AND REPAIR / REINFORCEMENT OF DAMAGED PERIMETER FENCE TO SECURE 130 HECTARES HOSPITAL LAND
LOT 5: RENOVATION OF ALL BLOCKS OF THE 35-YEAR-OLD HOSPITAL COMPLEX IN FORM OF ROOFS TREATMENT, RESTRUCTURING, REMODELLING, UPGRADE AND PAINTING TO ELONGATE THE LIFE SPAN OF THE HOSPITAL COMPLEX
LOT 6: REPAIR AND CONVERSION OF EXISTING GENERATORS AND VEHICLES TO CNG AND MINI TRUCKS, PATROL MOTOR BIKES TO ENHANCE COMMUNITY OUTREACH PROGRAMS AND INTERNAL SECURITY.
LOT 7: PROCUREMENT OF SPECIALISED OPHTHALMIC DIAGNOSTIC AND SURGICAL EQUIPMENT FOR RETINAL EVALUATION, VITREORETINAL SURGERIES AND CRYOTHERAPY ELIGIBILITY REQUIREMENTS

For the purpose of Evaluation, interested Contractors/Consultants should submit the following documents and also present Originals for Sighting during Opening of Technical Bids.

Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including Articles of Association, Form CAC 1.1 or CAC2 and CAC7. Business Name with Form BN1 is also acceptable for Services. With inclusion of Evidence of up-to-date Filing of Annual Returns at CAC.

Evidence of Company’s Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificates of all the partners in case of Business Name) for the last three (3) years valid till 31st December 2025.

Evidence of Pension Clearance Certificate valid till 31st December 2025 (this requirement is only applicable to bidders whose number of staff is 15 and above);

Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2025 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N75m and N250m);

Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2025.

Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31st December 2025 or valid Certificate issued by BPP Sworn Affidavit;

Disclosing whether or not any officer of the relevant committees of the The National Eye Centre Kaduna or the Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars; • That no Director has been convicted in any country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter;

That the company is not in receivership, the subject of any form of insolvency or bankruptcy proceedings or the subject of any form of winding up petition or proceedings.

That the required documents submitted are both genuine and up-to-date and that the hospital can verify the documents.

That none of the Directors of the company has ever been convicted by any Court of Law That the Company does not have existing loan or financial liability with any bank, other financial institution or third party that is classified as doubtful, non-performance, bad or whose repayment portion has been outstanding for the last four (4) months

Company’s Audited Accounts for the last three (3) Years (2022, 2023 and 2024), duly acknowledged by the Federal Inland Revenue Service (FIRS), Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the Project, including copies of their Academic/Professional Certificates such as COREN, QSRBN, ARCON, CORBON, etc.

Verifiable documentary evidence of at least three (3) similar projects (in size, nature & complexity) executed in the last five (5) years including Letters of award, Valuation Certificates, Job Completion Certificates and Photographs of the projects as applicable.

Category A (Works): List of Plants/Equipment with proof of Ownership/ Lease Agreement.

Reference letter from reputable Commercial Bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed.

Detailed Company profile to include the verifiable names, phone numbers, Curriculum Vitae and credentials of Technical and Management Staffs;

Category B (Goods): Letter of Authorisation from the Original Equipment Manufacturer listed in the Approved Policy for procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria;

Category C (Services): Lot S1; Evidence of firm’s current registration with the relevant regulatory bodies such as CMD, MDCN etc For Joint Venture/Partnership, Memorandum of Understanding (MoU)

should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner)

All documents for submission must be transmitted with a Covering/ Forwarding letter under the Company/Firm’s Letter Head Paper bearing among others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Numbers (at least two preferably GSM) and E-mail Address. The Letter Head Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page duly signed by authorised officer of the firm.

The EOIs/Technical bids should be orderly and well paginated with table of content and follow the order listed above, failure to comply with this directive will result in disqualification of the bidder.

D. SUBMISSION OF TECHNICAL BID DOCUMENTS (WORKS, GOODS,& SERVICES)

Completed EOI Technical documents should be Submitted in two Hard copies (Marked “Original” & “Copy”) with a Soft copy in PDF Format packaged in a sealed envelope clearly marked at the top right corner of each envelope stating the title of the project i.e. “TECHNICAL BIDS FOR LOT 1:” and at the reverse side the name of the company should boldly be written and address of the company at the reverse side and slide in the appropriate LOT Number Tender Box at The Centre’s Board Room, to be addressed to:

The submission should be made not later than 12:00 noon on Friday 27th June, 2025. Late application will be returned unopened. Please make sure you signed Bid Submission Register at the Point of Submission. The Procurement Unit of the National Eye Centre Kaduna will not be held responsible for misplaced or wrongly submitted Bids.

OPENING OF EOIs AND PRE-QUALIFICATION DOCUMENTS/ TECHNICAL BIDS
The Eol/Pre-qualification documents/technical bids will be opened immediately after the deadline for submission at 12:00 noon on 27th June, 2025, at The National Eye Centre Board Room.

NOTATION
Bids must be in English Language and duly signed by official authorised by the bidder All documents (sensitive and non-sensitive) submitted would be verified. Bidders are advised to ensure integrity of all documents submitted.

The evaluation of the received documents shall be strictly based on the requirements specified in this Invitation to Tender.

The submitted bids should be orderly and well paginated with table of contents particularly Technical Bids, any bid that are not in order shall NOT be accepted. Bidders should NOT bid for more than two (2) Lots. Incomplete Bids shall NOT be accepted.

Any bid without soft copy(ies) in specified format shall not be evaluated. All documents submitted will be subjected to Due Diligence Checks.

This publication is for information purpose only and does not constitute an offer by the Hospital to transact with any party for the project, nor does it constitute a commitment or an obligation on the part of the Hospital to award the contract to any contractor or agent.

The Hospital will not be responsible for any cost or expenses incurred by any interested company(ies) in connection with any response to this invitation and/or preparation or submission of the documents

The Bids opening will be covered by video recording (link will be sent to bidders who may not be able to attend on Request) with the presence of two (2) representatives of private sector professional bodies and civil Society Organisation in the areas of Anti-Corruption/Transparency.

The National Eye Centre Kaduna is not bound to pre-qualify/shortlist any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of Public Procurement Act (PPA), 2007.

Bid documents can only be collected in soft copies and Bidders can submit through Post (Courier Services);

Any False declaration upon verification will disqualify the Bidder. Only pre-qualified/shortlisted bidders at technical/Eol evaluation will be invited at a later date for collection of Tenders documents/request for Proposal;

All Technical Bids/Documents must have soft copy (as a single file in a Flash Drive) submission in addition to 2 hard copies to be submitted.
