INVITATION FOR TENDER
The Abia State Primary Health Care Development Agency, Umuahia hereby invites bids for tender from competent, reputable and interested Contractors/Supplier and Printers for the following jobs: (a) Renovation of BASIC PHC in Umumomalnta ward of Isiala Ngwa North LGA, OHANKU PHC of Ukwa East LGA, (b) Supply of Laptops (c)Printing of Health Facility Data Tools.
SCOPE OF WORK
RENOVATION OF FACILITIES: The contractor is expected to renovate the main buildings. Staff Quarters. Perimeter Fencing, Install Solar Powered Borehole and Solarization of the entire Facilities.
SUPPLY OF LAPTOP: The supplier is expected to supply 9 Pieces of HP Laptops.
PRINTING OF DATA TOOLS: The printer is expected to print HMIS GMP/Nutrition Register, ANC, OPD, General Attendance, PNC, Family Planning and Facility Monthly Summary forms.
TENDER/PRE-QUALIFICATION REQUIREMENT
Prospective tenders are required to submit their pre-qualification bid containing the under-listed documents In clearly marked envelope to the Office of the Executive Secretary within two weeks from this publication.
The opening of the bids fellow immediately after close of submission of the following documents:
(a) Evidence of payment of non-refundable appropriate tender fee. (b) Certificate of Registration with Corporate Affairs Commission (CAC). (c) Company profile including personnel and Equipment (d) A bid security in an amount not more than 2%of the bid price by (e) Details of verifiable experience on similar projects (f) Current Tax Clearance Certificate. (g) Current Finance status of the company. (h) Fully priced Bill of Quantities. (i) Certificate of Registration with Abia State Government (Due Process and Public Procurement Bureau) for facility renovation. (j) Technical Design (k) Certified true copies of Memorandum and Articles of Associated of the Company.
TENDER PROCEDURE
Completed tenders should be sealed in an envelope and clearly marked to indicate the project being tendered for to the Office of the Executive Secretary of the Agency before 12noon on the day of close offender. Please, note only those contractors that are pre-qualified and found suitable for the work will have their tenders considered for the award of contract for the project. Closing date for the submission of the tender is 2 weeks from the date of this publication and the opening of the tenders follows immediately thereafter in the conference Room of ABIA STATE PRIMARY HEALTH CARE DEVELOPMENT AGENCY.
