INVITATION FOR PRE-QUALIFICATION UNDER THE YEAR 2025 CAPITAL APPROPRIATION PROJECTS
1.0 INTRODUCTION
1.1 The Federal Medical Centre Daura (FMC) was established with the core objectives of providing accessible and affordable health care service delivery across the region. In fulfilling its obligation, for the Procurement of some necessary medical equipment for medical diagnosis as well as renovation of the existing medical facility that has been in existence since inception of General Hospital Daura some decades ago. These are Medical Devices that is used to diagnose ailment.
1.2 The Federal Medical Centre Daura in fulfilment of its obligation to provide quality and affordable Health care services hereby invites experienced and competent bidders for the following Projects.
2.0 SCOPE OF GOODS AND WORKS.

A-1 Provision and Installation of Solar Power at FMC Daura Temporary Site
A-2 Procurement of Office Furnitures, Fridges and Air conditions to FMC Daura
A-3 Procurement of office Computers
CATEGORY B WORKS.
B-1 Construction of Accident and Emergency at FMC Daura Permanent Site
B-2 Construction of Specialist Consulting Clinic at FMC Daura Permanent Site
B-3 Construction of Administrative Block at FMC Daura Permanent Site
B-4 Construction of Stores at FMC Daura Permanent Site.
B-5 Construction of General outpatient department
3 ELIGIBILITY REQUIREMENTS:
(a) Evidence of Certificate of Incorporation with the Corporate Affairs Commission (CAC) including form CAC1.1 or CAC2 and CAC 7;
(b) Evidence of Company’s Income Tax Clearance Certificate for the last (3) years (2022, 2023, 2024) valid till 31st December, 2025; turnover of N50 Million for category (A) lots except Category (B) which is minimum average annual turnover of N200 Million;
(c) Evidence of Pension Clearance Certificate valid till 31st December, 2025 (this requirement is only applicable to bidders whose number of staff is 15 and above);
(d) Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2025 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s);
(e) Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2025;
(f) Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2025 or valid Certificate issued by BPP;
(g) Sworn Affidavit disclosing whether or not any officer of the relevant committees of the Federal Medical Centre Daura. (FMCD or the Bureau of Public Procurement is a former or present director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
(h) Company’s Audited Accounts for the last three (3) years 2022, 2023, 2024; duly acknowledged and stamped by the Federal Inland Revenue.
(i) Reference letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
(j) Company’s profile with the Curriculum vitae of key staff to be deployed for the Project, including copies of their Academic/Professional qualifications such as SURCON, COREN, QSRBN, ARCON, CORBON, etc;
(k) Evidence of firm’s registration with relevant regulatory professional body(ies) such as CMC, COREN/QSRBN/ARCON/SURCON/ESVARBON etc.
l) For ICT Jobs: Evidence of firm’s current registration/ accreditation with computer professional council of Nigeria and National Information Technology Development Agency (NITDA).
(m) Verifiable documentary evidence of at least (3) similar jobs (in size, nature & complexity) executed in the last five (5) years and letter of awards, Valuation Certificates, Job Completion Certificate and Photograph of Projects, List of Plants/Equipment with proof of ownership/lease; for Category (B)
(q) For Joint Venture/Partnership, Memorandum of Understanding (MoU) indicating Lead Partner should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner).
Note: All documents for submission must be transmitted with a covering/Forwarding letter under the Company’s letter Head paper bearing amongst others, the registration number (RC) as issued by the Corporate Affairs Commission, contact address, telephone number (preferably GSM No) and email address. The letter head paper must bear the names and nationalities of the directors of the company at the bottom page, duly signed by the authorized officer of the firm.
5. METHOD OF SUBMISSION
1) Bids Must be prepared in English language and Signed by an official authorised by the bidder;
11) Two (Original and copy) of Technical documents for all categories must be in a sealed envelopes clearly marked with name of the project and lot number, and addressed to.
The Medical Director, Federal Medical Centre Daura, Katsina Road Opposite local Government Sectreriat PMB 1051. The reserve of each envelope shall bear the name and address of the bidder and submitted in a Tempered Proof Tender Box on or before 12.00 noon on Wednesday 15 October 2025 at the office of Head of procurement Department opposite central mosque Federal Medical Centre Daura Nigeria.
6 OPENING OF PRE-QUALIFICATION
The Pre- Qualification Documents will be opened immediately after deadline for submission at 12:00 noon on 15 October, 2025. at the FMCD Conference Room, please ensure that you sign the Bid Submission Register at the Head of Procurement’s Office as the FMCD will not be held liable for misplaced or wrongly submitted bids.
7 GENERAL INFORMATION
a. Bids must be in English Language and signed by an official authorised by the bidder;
b. Bids submitted after the deadline for submission would be returned un-opened;
c. Bidders should not bid for more than two (2) lots
d. All cost will be borne by the bidders;
e. The Technical Bids will be opened in the presence of Bidders or their representatives, CSOs in the areas of Anti-Corruption and Transparency and Private Professional Bodies; and will be covered by video recording; invitation link will be sent to bidder who choose to join
f. Only pre-qualified bidders at technical evaluation will be invited at a later date for financial bids opening, while the financial bids of un-successful bidders will be returned un-opened;
g. Federal Medical Centre Daura. (FMCD) Daura is not bound to prequalify any bidders and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with S.28 of the Public Procurement Act 2007.
