INVITATION TO TENDER/INVITATION FOR PREQUALIFICATION
1. INTRODUCTION
The National Obstetric Fistula Centre Benin hereby invites all interested and eligible suppliers/contractors with relevant experience to bid and apply for prequalification for tendering for the execution of the following projects to be funded with the FGN 2025 Appropriations.
2. SCOPE OF GOODS AND WORKS

The invitation to tender applies to the following projects:

3. ELIGIBILITY REQUIREMENTS
Interested firms should submit the following eligibility requirements:
a. Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including Form CAC1.1 or CAC2 and CAC7; Certificate of Registration of business name with form BN1 is also acceptable;
b. Evidence of Company’s Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificates of all the Partners in case of Business Name) for the last three (3) years (2022, 2023, 2024) valid till 31st December, 2025;
c. Evidence of Pension Clearance Certificate valid till 31st December, 2025 (this requirement is only applicable to bidders whose number of staff is 15 and above);
d. Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2025 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above);
e. Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2025;
f. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2025 or valid Certificate issued by Bureau of Public Procurement;
g. Sworn Affidavit:
– disclosing whether or not any officer of the relevant committees of the National Obstetric Fistula Centre Benin or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
– that no Director has been convicted in any country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter;
– that the company is not in receivership, the subject of any form of insolvency or bankruptcy proceedings or the subject of any form of winding up petition or proceedings;
h. Company’s Audited Accounts for the last three (3) years – 2022, 2023 & 2024;
i. Reference Letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
j. Verifiable documentary evidence of at least three (3) similar jobs (in size, nature & complexity) executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects;
k. For Supply of Medical and Laboratory Equipment: Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria;
l. For Supply of Other Equipment: Letter of Authorisation as representative of the Original Equipment Manufacturers (OEMs);
m. Only Companies included in the National Automotive Design and Development Council’s approved list of local manufacturers of vehicles or their authorised representatives should bid for the Vehicles;
n. Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications such as COREN, QSRBN, ARCON, CORBON etc.;
o. Works: List of Plants/Equipment with proof of Ownership/Lease Agreement;
p. For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner).
Note: All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorised officer of the firm. These documents are to be arranged in the order in which they are listed.
4. COLLECTION OF BIDDING AND PREQUALIFICATION DOCUMENTS
Prospective bidders are to collect the Standard Bidding and Prequalification Documents from Administrative Building, National Obstetric Fistula Centre, Benin City starting from the date of publication of this advert between the hours of 9:00 am and 4:00 pm. Evidence of payment of a non-refundable fee of N10,000.00 (Ten thousand naira) only per Lot made through REMITA PLATFORM to National Obstetric Fistula Centre Benin at any reputable Commercial Bank in Nigeria must be presented before collection of the bidding/prequalification documents.
5. SUBMISSION OF BIDS AND PREQUALIFICATION DOCUMENTS
• For Lot 1-4, Two (2) hard copies of the technical bid and two (2) hard copies of the financial bid with its soft (electronic) copy in Microsoft Excel format, should be packaged separately in sealed envelopes and clearly marked as “Technical Bid” or “Financial Bid”. The two envelopes are to be enclosed and sealed in one large envelope addressed to The Medical Director, National Obstetric Fistula Centre, Benin City, Nigeria, and clearly marked with the Lot Number on the top right corner. The reverse side of the large sealed envelope should bear the name and address of the bidder. This should be deposited in the tamper-proof tender box in The Procurement Unit, Administrative Building, National Obstetric Fistula Centre, Benin City not later than 12:00 noon, Monday, 26th May 2025.
• Applicants for prequalification are to enclose copies of the eligibility requirements listed in section 3.0 above, and copies of other prequalification documents in a sealed envelope addressed to The Medical Director, National Obstetric Fistula Centre, Benin City, and marked “PREQUALIFICATION FOR TENDER” on the reverse side. This should be deposited in the tamper-proof tender box in The Procurement Unit, Administrative Building, National Obstetric Fistula Centre, Benin City, not later than 12:00 noon, Monday, 26th May 2025.
• Bidders and applicants are to ensure that they sign the bid and application (prequalification) submission register at the point of submission. Bidders should obtain a bid submission receipt, as the Centre will not be held liable for misplaced or wrongly submitted bids. Bids and application received after the indicated deadline shall not be considered.
6. OPENING OF TECHNICAL BIDS AND PREQUALIFICATION DOCUMENTS
The technical bids and prequalification documents shall be opened immediately following the deadline for submission at 12:00 noon, Monday, 26th May 2025, at the Administrative Building, National Obstetric Fistula Centre, Benin City. The documents will be opened in the presence of bidders/applicants or their representatives, registered Civil Society Organisations (CSOs) in the areas of anti-corruption and transparency, and relevant private professional bodies.
7. GENERAL INFORMATION
a. Bids must be in English Language and signed by an official authorised by the bidder/applicants.
b. All costs will be borne by the bidders/applicants.
c. Only bidders whose technical bids pass through the technical evaluation will be invited in writing to the opening of the financial bids on a later date. The financial bids of unsuccessful bidders will be returned unopened. Successfully prequalified applicants for Lot 5 shall be informed in writing, and invited for the formal tendering procedure.
d. Prospective bidders should note that enclosure of Technical and Financial bids in one and same envelope which will result to exposure of the financial bid at the time of technical bid opening will result to automatic disqualification.
e. Submissions that do not meet up with the requirements specified above shall be rejected.
f. Evaluation of bids and prequalification documents shall be strictly based on the requirements specified above and the evaluation and qualification criteria indicated in the Revised Standard Bidding Document.
g. The National Obstetric Fistula Centre Benin, is not bound to consider any bid or application and reserves the right to annul the procurement process at any time in the public interest without incurring any liabilities to the bidders or applicants in accordance with Section 28 of the Public Procurement Act 2007.
h. Due diligence exercise shall be conducted to verify all the claims made by bidders/applicants.
